MERKNAD TIL RAPPORT VEDR. INNFØRING AV ROTAVIRUSVAKSINE I BARNEVAKSINASJONSPROGRAMMET.
Som angitt i de fortløpende diskusjonene i arbeidsgruppen og i e-post til arbeidsgruppens leder 30.09.10, har Landsgruppen av helsesøstre NSF (LaH NSF) hatt behov for å avklare endelig standpunkt til innføring av rotavirusvaksine i Barnevaksinasjonsprogrammet. Vi har opplevd en klar forventning om å konkludere med en positiv innstilling til innføring av denne vaksinen, noe som i seg selv ikke er uproblematisk når en arbeidsgruppe skal foreta en objektiv vurdering.
Etter en grundig vurdering, både i LaH NSF og i Norsk Sykepleierforbund, har vi kommet til at vi ikke ønsker å støtte arbeidsgruppens forslag. Som en følge av dette, ønsker vi i rapportens kapittel 10 (og kap. 1 som er en kopi av kap. 10), tilføyd følgende:

Vår begrunnelse for dette er i korte trekk:
Vår oppfatning er at hensynet til barnets beste skal veie tyngre enn de evt. samfunnsøkonomiske gevinstene. Vi vet at det å ligge på sykehus er en påkjenning for barn (og foreldre), med risiko for senere psykiske problemer1. Vaksinasjon innebærerimidlertid også en stressituasjon for både barn og foreldre. Her må man vurdere 900sykehusinnleggelser årlig opp mot nesten 185.000 vaksinesituasjoner årlig, og hva somer ”barnets beste”.
Vi kan ikke se at dokumentasjonen på samfunnsøkonomisk gevinst er så entydig at det er et godt nok argument for innføring av vaksinen.
Vi mener man ikke kan vurdere denne sykdommen og denne vaksinen isolert. Vi vet at det foreligger og vil komme forslag om andre vaksiner som ønskes tatt inn i Barnevaksinasjonsprogrammet, eks. hepatitt B. Undersøkelser i regi av FHI har vist at småbarnsforeldre er positive til vaksine, men det er ikke spurt spesifikt om holdninger tilrotavirusvaksine kontra andre vaksiner. Vi opplever større skepsis både blant foreldre og helsesøstre til å innføre stadig nye vaksiner. Debatten rundt innføring av HPV- vaksinen og den siste tidens dokumentasjon på alvorlige bivirkninger av svineinfluensavaksinen har forsterket dette. For å lykkes med en vaksinestrategi er man avhengige både av motiverte foreldre og helsesøstre, sistnevnte er de som både skal informere, motivere og gjennomføre alle vaksinasjoner i Barnevaksinasjonsprogrammet.
Vi er også spørrende til om denne vaksinen bør prioriteres innført med de betydelige kostnadene det innebærer, sett i lys av andre alvorlige helseutfordringer blant barn og unge, eksempelvis overvekt/fedme, psykiske helseplager osv.
Vi spør oss om det hadde det vært rimeligere og mer hensiktsmessig med en systematisk opplysningskampanje i forhold til smitte, håndhygiene osv? Bedre håndhygiene og enkle smitteverntiltak vil ha effekt også utover forebygging av rotavirus, og vil slik kunne gi en folkehelsemessig og samfunnsøkonomisk gevinst. Erfaringer fra svineinfluensapandemien tyder på at dette kan være en klok strategi.
Produsentene av vaksinene har underveis i vurderingsprosessen redusert prisen på vaksinene betraktelig, og hevder at vaksinasjon dermed også er kostnadseffektivt i et helsetjenesteperspektiv. Vi tenker det er viktig å ha de sterke kommersielle interessene i mente når en konklusjon skal trekkes.
Vi er klar over at det er utenfor arbeidsgruppens mandat å vurdere etiske/globale forhold relatert til innføring av denne vaksinen i Norge. Vi har imidlertid sett det riktig å diskutere etiske problemstillinger knyttet til at Norge som et av verdens rikeste land skal innføre vaksine mot en sykdom som etter vår vurdering er et begrenset problem her til lands. I en rekke fattige land er rotavirusgastroenteritt et reelt og alvorlig folkehelseproblem og medfører hyppig død blant sped- og småbarn.
Et viktig etisk aspekt for oss, har vært at innføring av vaksinen i Barnevaksinasjonsprogrammet kan bidra til utjevning av sosial ulikhet i helse, ved at vaksinen tilbys alle barn kostnadsfritt for familien. Det vil imidlertid være et argument som er gyldig i forhold til alle vaksiner som vurderes inntatt i Barnevaksinasjons-programmet.
Vi foreslår også en tekstendring i kap. 8.3 hvor det påstås at sykdommen er et folkehelseproblem. Et folkehelseproblem er så langt vi har kunnet finne en presis definisjon på, når en lidelse rammer minst 1% av befolkningen. Ut fra fremlagt dokumentasjon i rapporten, kan vi ikke se at dette er tilfelle hva gjelder rotavirusinfeksjon.
